I was thinking of cases along the lines of the Spanish athlete in the 80's who's name I can't remember. She was disqualified and banned after being gender tested (the test at the time was checking chromosomes) and found to be XY even though to all visible intents and purposes she was female, was born female, considered herself to be female. The theory about her was that the gene that triggers development of male sexual characteristics upon the bombardment of testosterone in the womb at some point wasn't turned on, so although she was XY continued to develop as female.
