So, to recap, the humour lies in the premise of the joke. The man calls the dentist when he is experiencing pain in his tooth and asks what time he can make an appointment. The quirks of the spoken English language allows for the delicious juxtaposition of the time spoken aloud and the symptoms the patient is experiencing.

Actually...this doesn't work when written down and is best spoken aloud.
