ipod bullshit (1 Viewer)

so i have to send my ipod back to apple (making it the third ipod ive returned) but i cant find the old service request form on the support page. maybe im just being a tard but doesnt seem to be there. and im not sending it back through the retailer as theyre now suggesting (o2 took 4 months with it the last time).
cunts.
anyways, any help would be only deadly
