I vaguely remember this from the early-mid 90s.

Possibly. It's the one where the guy from E.T. is wheelchair-bound and gets his thrills from being cuckolded, on a ship. Lots of flashbacks.
The female leads kiss. Hugh Grant looks terribly stressed for most of it.
