I'm not sure on the back story on this but the heavily muscled/tattooed guy is Jeff Monson - a Mixed Martial Arts fighter and political/anarchist activist.

i wonder how set up it is..Monson though a huge man isn't that tall (about 5' 9") - the angle/perspective makes him look bigger.

Also I wonder what the photographer was standing on.

I'd say handheld over the head with a wide lens on. Newer cameras will have live preview deelys on them so you can compose with the back of the camera like a cheapo digital camera. Or the photographer could have just guessed, i've done that myself a good few times.

Think you have to lock your focus before you switch to live view mode. I'd say he took a few holding it over his head and went with that one.

That's quite possible. He could have focused off dude's back, set an aperature of f8 or more and shot it, the depth of field isn't all that big. You're probably right though.
