According to a dentist mate, a 'clearance' is A Bad Thing - psychologically anyway.

Clearance = extraction of all teeth.

I hate the dentist too but it just has to be done sometimes. I remember fondly one dentist who was rough as fuck and would throw drill bits across the room in a fit of rage because they were too blunt or something. Wasn't a very relaxing experience.
