Can you help me Identify this Graphics card? (2 Viewers)

Got a free Dell desktop/work station from a friend's business that was upgrading.

Well anyway - I reformated it and installed a new OS (Win 7 pro) but I can't get drivers for the Graphics card.

It was made to fit in a very low profile case. Its outputs are DVI and S-video.
